@RoboDocX I had a similar case - I went for a robotic pyelolithotomy and cystolithotomy and intraoperative transperitoneal (robotic) flexible URS. URS wasn’t completely successful but good enough for stent removal. Patient needed a second URS for removal of the remaining stones.

Let's talk strategy! 65 year old male with retained ureteral stent > 5 years. Complete staghorn (only partially shown), encrusted ureteral stent and 8 cm bladder stone. Kidney 25% by mag3. Patient wants to try to rehabilitate this kidney. What's next in your algorithm?

@RoboDocX I like what Dr. Badri @AnandBadriMD stated. I would go SP at level of 11th rib transabdominal and do kidney portion first and then go to slight T-burg, and do cystotomy. Going retro would be difficult unless your really good at the SARA by Dr. Crivellaro @SimoneCrivella2 .
