Not every stroke patient should walk out with ‘aspirin forever. In lacunar stroke, intracranial stenosis, or heavy microbleed burden, cilostazol can quietly beat aspirin on both ischemia and bleeding. Cilostazol is a PDE‑3 inhibitor that does more than just block platelets. It reduces aggregation, dilates arteries, and protects the endothelium, which is exactly where things go wrong in small‑vessel disease and intracranial stenosis. Compared with aspirin, trials and meta‑analyses show fewer recurrent ischemic strokes and significantly less intracranial hemorrhage, with headache and palpitations as the main trade‑offs. Clinically, think cilostazol for: Lacunar strokes and cerebral small‑vessel disease Symptomatic intracranial arterial stenosis Patients loaded with cerebral microbleeds or at high ICH risk Long‑term dual therapy (with aspirin or clopidogrel) when you want ischemic protection without the usual bleeding penalty Avoid it in heart failure, cardioembolic stroke, or active bleeding.
