Case of the Day - 191 - Effortless Segmentation - The AI We Want 41-years old treated for sigmoid colon carcinoma came for a PET/CT. The video describes the use of the Totalsegmentator plugin in Osirix MD that can segment the liver in less than 5 minutes, in the background. I then added the liver vessel module superimposed on the segments. This is the kind of AI we want, something that makes our lives easier and simpler, within our existing workflows. The "E" Sign of a Bipartite Medial Cuneiform 39-years old with chronic foot pain. The patient came for a CT. The axial (A) and sagittal (B) show a bipartite medial cuneiform with cystic areas at the pseudoarthrosis. The 3DCT shows the "E" sign created by this configuration. Post 02 - Progressive Pulmonary Fibrosis (PPF) and Biologic Behavior 70-years old with a fibrosing ILD that shows a usual interstitial pneumonia (UIP) - like pattern on the left, but a three-density sign on the right suggesting this could be a bronchiolocentric interstitial pneumonia (BIP) pattern, suggestive of fibrotic hypersensitivity pneumonitis, if there was no underlying etiology. This patient however has scans from 2016 spanning 10 years showing progression (PPF). The post shows 8 of these scans together to understand the biologic behavior of this disease over 10 years. Heat: The Invisible Barrier to Long-Term Health and Longevity Increased temperatures increase physical inactivity in low and middle income countries. Heatwaves increase the risk of death, cardiovascular and otherwise with an exponential increase above a level specific for the region concerned. Compound heat (daytime and nighttime both) is the worst. Green covers can significantly make a difference in reducing heat-related deaths. We need to make a personal effort to reduce our exposure to extreme heat, both short-term and long-term. Case of the Day - 186 - Spina Ventosa is Not Only Dactylitis...Any Bone Can be Involved 11-years old with fever and swelling of the sternum and neck nodes. MRI showed an expansile osteolytic lesion in the manubrium sternum with supraclavicular and mediastinal adenopathy. There was worry about neoplasm and a CT guided biopsy was done, which revealed histopath and culture positive tuberculosis. The video describes the case, the "ballooned-out" manubrium, the concept of spina ventosa, pictures from the 18th century showing spina ventosa in different bones of the body including the craniofacial skeleton and images of different bones show spina ventosa. In short, "spina ventosa", though synonymous with tuberculous dactylitis in current times, can actually affect any bone in the body...phalanges and metacarpals and metatarsals are just the more commonly involved bones.
