LHON LHON is a mitochondrial optic neuropathy characterized by: Subacute painless central vision loss Bilateral involvement (often sequential) Maternal inheritance The clue in this case is the history of affected maternal relatives, which strongly suggests mitochondrial inheritance. Genetics and Inheritance LHON is caused by mutations in mitochondrial DNA (mtDNA). Important features of mitochondrial inheritance: Transmitted only through the mother Both males and females can be affected However, males are affected more frequently The three most common mtDNA mutations are: 11778 mutation (ND4 gene) – most common and worst prognosis 14484 mutation (ND6 gene) – better visual recovery 3460 mutation (ND1 gene) Because mitochondria are inherited from the ovum, the disease passes through the maternal lineage. Pathophysiology The mutation leads to mitochondrial respiratory chain dysfunction, particularly affecting complex I. A 16-year-old boy presented with a persistent homonymous right inferior quadrantanopia visual field defect (A) 1 year after a ruptured left thalamic arteriovenous malformation, for which he was treated with radiation due to high-risk location precluding surgical intervention. Magnetic resonance imaging with axial flair sequence showed hemorrhage of vascular nidus in the left thalamus (B). Macular spectral-domain OCT (C-D) revealed right-sided ganglion cell thinning with corresponding right-sided schisis-like cavities at the inner nuclear layer, likely representing retrograde axoplasmic degeneration due to dysfunction in the lateral geniculate nucleus. A 24-year-old man presented with sudden reduction in vision in the right eye (3/60) after physical exertion while playing badminton. Examination revealed a vessel traversing the vitreous cavity attached anteriorly to the posterior lens capsule with localized cataract (A, arrow) and extending toward the iris (A, arrowhead) and posteriorly toward the optic disc (B, arrow) with associated vitreous hemorrhage. A diagnosis of persistent primary hyaloidal vessel was made and observation was recommended. Blood cleared and his vision improved (6/9) revealing the vessel attachment from the optic disc (C, arrow) to the lens capsule (D, arrow).
