Jesus fuck, at the end of BBC’s Ambulance they announced one of the paramedics died shortly after a short illness. After all they help, care, and love she gave 😞

@DrHelenAnderson @louisebrady17 You would think in a modern day world with technology this would simple but it’s impossible. Pathways for patients fail and ultimately fall through the net. And when patients get discharged from hospital, the continuity of care pretty much stops.

This is really sad. Continuity of care is so important. When I used to work in general practice we would regularly book follow up ourselves for patients, but the system and capacity issues no longer allow for this. As well as the negative effect on patients it can lead to moral distress for clinicians who can’t provide the standard of care they want
