Thank you to everyone for my birthday wishes yesterday! I ate lots of cake and now I feel guilty about the amount of cake I ate. So all in all, a pretty standard birthday with post birthday guilt.

As brilliant as our transfer window has been, Geuhi is a big loss imo. Defence is our big weakness and with Konate injured/inconsistent and Leoni inexperienced, we only really have VVD Really hope we get Geuhi over the line in Jan otherwise winning anything might be tough #LFC

As much as I love being back in GP, I have not missed getting a cold every other week. I feel so unwell and I expect massive amounts of sympathy for getting ill in the line of duty or I'll be accusing everyone of lack of empathy.

@Medic_Mermaid There's always that optimism of "well it's a bank holiday weekend surely people have better things to do then come to hospital right?" Big nope.

Had a mild panic and quickly checked HealthRota to make sure I wasn't on call this bank holiday weekend. Then I remembered that I'm back in wonderful GP land where I never have to work on weekends or bank holidays. Good luck to everyone on call! You'll be in my thoughts

I really hope that this was just a case of miscommunication between the patient and the person at the A+E reception that day and maybe they didn't read my letter. I only posted this because It's just scary to think that someone in A+E saw these symptoms and discharged them home.

@richard_morland I think triaging patients is completely fine in both GP and A+E but I think if non clinical staff are going to overrule a clinical decision they should check with a clinical staff member first. I'm sure they usually do, this is just a terrifying example of someone who didn't.

@jaydbint Remember consultants and a clinical director have colluded in allowing an A+E receptionist to triage patients How carefully are GP receptionist monitored to ensure urgent cases are seen quickly?

I feel like as doctors we're so used to exam culture and looking for tricks in questions or complicated explanations because "it can't be that simple can it?" Whereas in reality, it's usually the simple explanation which is the correct one.

As a Gynae SHO I had a referral from A+E about a girl with recurrent vaginal bleeding associated with abdo pain which happened monthly and self resolved. The doc thought it was weird because she didn't have a coil or any contraception. It was her period. She was on her period.
