@ABsteward Thanks for sharing this very interesting meta analysis on letermovir prophylaxis in SOT- I feel like valganciclovir may be joining the vancomycin , poly B, and aminoglycosides club , as a medication that we will be glad not to have to use routinely

Beyond valganciclovir! 🆕⚡🟢Meta-analysis of letermovir’s safety and efficacy in cytomegalovirus prophylaxis after solid organ transplantation 6 studies: 1 RCT + 5 retrospective or matched cohort studies N= 1,413 recipients (526 LTV; 887 VGC) LTV significantly reduced CMV DNAaemia during prophylaxis (RR = 0.33, 95% CI 0.18–0.59; I² = 0%), with comparable CMV disease incidence (RR = 0.87, 95% CI 0.41–1.88). Post-prophylaxis CMV infection, graft rejection, graft loss, and mortality were similar between groups. LTV showed numerically lower rates of neutropenia, leukopenia, and G-CSF use, reflecting improved haematologic safety, although between-study heterogeneity was substantial #IDXposts link.springer.com/article/10.118…

This is important study! I think it's time to raise awareness about the importance of monitoring for low immunoglobulin levels and repleting patients to prevent infections- this approach may have additional benefits in preventing alloimmunity as well @AST_IDCOP @AST_KPCOP

@amjtransplant This is an important study on anti HLA antibody appearance after blood transfusion in Kidney Transplant recipients. Interesting finding that HLA class I antibodies were much more common than HLA class II @AST_KPCOP

I agree with you (and Brad) My 2 cents : -There are «bad» I.D. docs out there. (as in any other specialties) -For many (most?) doctors, it's hard to ignore a «positive» test (PCR + EIA -) Bottomline, C.diff PCR+ / EIA - is tricky p.s. I'm still a big supporter of 2-tier C.diff test
