Siobhan Huggins at CoSci, Las Vegas. Lymphedema is a devastating and often lifelong side effect of cancer treatment. Most of the effort goes into managing it after it appears. Siobhan is asking a different question: could metabolic health before treatment change the risk? When excess electrons back up in the electron transport chain, reactive oxygen species are produced, which can blunt insulin signaling. But none of us are the “average.” Emily talks about microsampling. Small, simple blood samples people can collect at home and send in regularly. Dr. Annette Bosworth on stage at CoSci, Las Vegas. Sequenced Metabolic Repair @AnnetteBosworth presents a striking case of severe metabolic dysfunction. Type 2 diabetes, prior cancer treatment, obesity, edema. HbA1c 10.9%. Triglycerides 547. Standard approaches failed. Even a 21 day water fast did not produce meaningful ketosis. Two years of high dose tirzepatide had no effect. Dr. Boz then shifted the strategy: repair metabolism first. Nutritional ketosis to restore mitochondrial function, then reintroducing GLP-1 therapy at a tiny micro dose. At #CoSci in Las Vegas @DominicDAgosti2 Brain Energy and Brain Injury: Considerations for Older Adults We tend to associate traumatic brain injury with athletes or the military. In reality, it is most common in older adults. After 65, even relatively small head impacts can have serious consequences. Dr. D’Agostino framed brain injury as a problem of brain energy and resilience, not just the impact itself. Protecting brain metabolism, maintaining strength and balance, and supporting metabolic health may reduce risk and improve recovery as we age. @realDaveFeldman @realCSF

But small changes on CCTA are close to the limits of what the scan can reliably measure. When you account for that noise, many trials actually show increases in coronary calcium and plaque volume. When glycogen and fat storage are full, excess fuel spills into the liver, pancreas, muscle, and bloodstream. That overflow drives insulin resistance, rising triglycerides, and chronically elevated fasting insulin, often long before glucose rises. The key concept: the personal fat threshold. The gut microbiome is part of the equation. Hormonal shifts can change the microbiome, and the microbiome can influence how those hormonal changes are experienced. That may help explain why some women continue to struggle with symptoms despite standard approaches. At CoSci Las Vegas: the LMHR panel with Dr. Nadir Ali, Dr. Nick Norwitz, Dr. Bret Scher, and Dave Feldman. The Lean Mass Hyper Responder phenotype has become one of the most debated topics in metabolic health. These are people who are metabolically healthy on low carb or ketogenic diets, yet show very high LDL, alongside high HDL and low triglycerides. That pattern does not fit the traditional cardiovascular risk model, which is why it has sparked so much discussion.
