Fascinating, and great job by all - 2 large trials that answer an important question. Glad that we reserved a decision on a broader rollout of IO in our service (from just CCPs to all paramedics) while awaiting the results of PARAMEDIC3. #researchmatters

2 RCTs testing IV vs IO vascular access in OHCA patients just published in the @NEJM and concomitantly presented at #RESUS24 @ERC_resus ➡️ No difference in 30d survival in both trials ➡️ Higher rate of ROSC with IV access in the #PARAMEDIC3 trial 🇬🇧 nejm.org/doi/full/10.10… 🇩🇰 nejm.org/doi/full/10.10… Congratulations to the authors for randomizing 6082 + 1506 patients to answer this important question!

One of the biggest reasons I moved to North America was to enjoy all the hiking it has to offer. This trail in Yosemite took 8 hours, gaining 1296m elevation over 30km and all in 42°C heat. Life is for living ⛰️

@em_bulance You had the courage to try. It’s ok that it didn’t work out the way you had hoped or planned. Always put yourself first and do what suits your needs and your personal values. Never feel like you have to explain yourself to anyone! ✌️ It’s an adventure chapter in your life story📚

* side note, I’d like to add if you have a family or partner and are doing the move together I think your situation would be totally different. This is also just my perspective and personal opinion, I am not trying to dissuade anyone from making the jump.

1/3 It’s been a journey to say the least but I think this one is coming to an end. I personally believe Canada is behind in terms of paramedicine, I’ve gained scope but lost autonomy. I struggle to work in an environment where paramedics aren’t treated as equals with nurses.

Paramedics of all settings, I implore you, if you were sending a patient to ED please do not tell them they *will* receive an investigation/treatment or be seen by X specialist. I encounter this often and it can lead to conflict. I suspect same is true for referring back to GP.

If anyone knows of anyone or any companies hiring in Toronto let me know. Customer service/barista/retail etc. I think I’m done with all things paramedic in Canada now

Remember when CPGs were gospel and we never (really) questioned them? Well, this article by @BFlanagan73 and colleagues reminds us again, just how important it is to appraise the evidence. Fascinating read 👀👇 journals.sagepub.com/doi/10.1177/27…

3/ The behaviour of individuals under pressure in this instance did not need better protocols What was needed was for paramedics to operate within a community of practice where ethical decision making and patient advocacy were a cornerstone of professional culture

@em_bulance It’s honest, and honesty is so valuable. Thank you for sharing your experiences, even more so since it hasn’t been all roses. And giving something new a go, is never an embarrassment - it is only ever brave.

I was a confident medic in London. But hearing colleagues make jibes about me here is upsetting. I am in a new country, with new medications, new protocols and I am trying my best. I don’t feel welcomed and quite honestly don’t think I’ll stick this out.

So it’s time to start planning, researching and prepping our next online course! We’ve wanted to cover ECGs for a long time. Let us know if you think this is a good choice for 2024, or even suggestions for a different topic…

This was one of the best articles I’ve read in a while It has deeply impacted and changed how I see my role in providing emergency mental healthcare. I encourage you, my friends, to read it too. Thank you @drrachelgibbons cambridge.org/core/journals/…

When NOT to stop resuscitation Sometimes we stop too early. Sometimes we go on too long. How do we pick the ones we should go the extra mile on? This 6 min excerpt from a talk really comes from the heart This is the concept that drives everything I do youtu.be/HiIsGmoHgpE
