@JackWestMD @BrendonStilesMD Big fan the term “aspirationally resectable” And, agree. Need to focus on meeting current standards for high quality lung cancer surgery. But as a field, we should still have certain people pushing the boundaries. Doing so shouldn’t distract from high quality surgery, everywhere

@BrendonStilesMD I'm not saying that you or the Altorkis, Spicers, & Swishers of the world can't use your good judgment to say 3B MAY be resectable, but saying we should routinize broadening to "aspirationally resectable" for surgeons who do CCY same day as occasional lung surgery is a BAD idea.

Almost all tumors are technically resectable - the question is “should we resect?” Perhaps more so than converting tumors, we need to convert physicians with an “unresectable” first mentality to a “resectable” mentality. #lcsm #TexasLung23

Thank you for sharing your experience! Hearing my patients’ stories over the years during surveillance has led me to believe (even though i cannot prove) that PFTs have value in risk-stratification, but they do not predict patient experience.

@FarhoodFarjah YES! My PFT's were fine post lobectomy but I was easily SOB when playing sports w/my 4 kids. AND there's only a finite amount of lung - I lived that horror/reality w/my aunt, whose 3rd stage I killed her because she didn’t have enough lung capacity to survive another surgery!

In fellowship our pathologists would regularly and it would help guide decision making. Particularly with STAS (though recognizing the difficulty to call it on a frozen specimen)

AUDARA update with longer F/U but OS data still withheld ascopubs.org/doi/pdf/10.120… Interesting👀 Brain mets incidence starts to rise AFTER 3 years osi is up in stage II/IIIA while in control group it plateaus at ~10%. So does osi really only delay recurrence?🧐 Do we need MRIs???

Importantly, for clinical relevance, we show that we can predict recurrence after surgery in a cohort of Stage 1A-B lung cancer patients by using machine learning and as a result can inform on adjuvant treatment strategies. We don’t need 35 markers- we can do this with just 6! 🙌🏼
