Friends and Colleagues, 25 years ago, I began my journey as a member of TTS. Over the past decade, I have had the privilege of serving as LatAm Counselor, Treasurer, and Vice President—roles that have shaped my perspective and deepened my commitment to this community.

In our recent editorial “Confidence Without Wisdom: The Dunning-Kruger Problem in Modern Surgery” we discussed how confidence, even though indispensable in surgery, if misaligned, it may place patients and professionals at risk. •Appreciating the perils of an overinflated confidence - especially early on in a surgeon’s career; •understanding how such behaviours are acquired and reinforced in a system that emphasises decisiveness, productivity and visible individual achievement; and •learning how it may be recalibrated under proper mentoring and honest self-reflection are essential for promoting patient safety, professional development, and ethically grounded surgical decision-making. @DimitriosMoris #cognitive_bias #dunning_kruger #self_assessment #surgical_judgment

In our recent commentary in the journal Transplantation, “Transplant Surgery: Sustaining a Workforce at a Critical Juncture,” we address an urgent reality: while abdominal transplantation remains one of medicine’s greatest successes, the specialty itself faces growing workforce instability. Declining trainee interest, unpredictable 24-hour call structures, misaligned productivity metrics, and promotion pathways that fail to reflect the true scope of transplant work all threaten long-term sustainability. We outline three structural priorities: • Redesign clinical operations to improve predictability — including greater use of machine perfusion to enable more daytime scheduling. 
• Modernize fellowship training and strengthen early-career support. 
• Realign academic promotion metrics to recognize clinical complexity, applied innovation, and systems leadership. This is not simply about surgeon wellbeing. A stable transplant workforce is fundamental to organ utilization, equity, and public trust. Sustaining transplant surgery requires structural reform — now. What is your opinion on that?

The use of older (>50years) DCD liver grafts in USA is increasing exponentially. From only 3 grafts in 2010, it went to 251 in 2022 and 1,595 in 2024. Now, older DCD is is the fastest growing donor category in usa. Advanced perfusion technologies (ex-situ machine perfusion and NRP) are game-changing and will be even more impactful in the next few years after broader adoption.
