There is a massive loophole in the medical system, and it is a patient's absolute best friend. It is the patient portal. When a doctor dismisses your symptoms or refuses a test, you do not have to argue or cry in the exam room. The exam room is designed to put them in a position of power. Elon Musk doesn't "have" a trillion dollars. It's just market cap on paper. He can't spend it. If he tried to sell his SpaceX stock, he would get much, much less for it. Market cap is pretty pointless if only 5% of a company is traded.

If you can’t tax billionaires and trillionaire because “its unrealized gains until they cash out,” then stop letting them leverage that wealth for loans, as collateral, or as equity.
If it’s not real enough to tax, it shouldn’t be real enough to leverage.
This is why we say the system is rigged in favor of the super wealthy. Abolish billionaires, tax them out of existence, protect working people.

There are actually plenty of ways to engage with mediums wrong
You can watch a movie on 5x speed, skip around scenes, play it in reverse
You can read a book in reverse, read the summary and that's it
Most mediums do indeed require bare minimum structure of engagement
