1/2 We are saddened to hear of the sudden passing of friend and colleague Nigel Hullah. Nigel worked very closely with our Dementia Team and leaves a remarkable legacy, having been a huge presence in the national and regional dementia programmes.

Here is one for you @HelenhowsonBC I’ve been the staff member trying to figure out why a pt is not on the list . I remember maybe @amcunningham sharing a similar story on X of a ‘Not an appointment letter’? Untrusted assessment, over checking, #sillyrule = mega-failure demand
Andy Ware@ACRWare

@AlexMorrish @JohnWSeddon Likely a #sillyrule - to determine your urgency your notes are needed, but can’t be released without an appointment. Creating one automatically prints a letter which is normally destroyed . Some are accidentally sent and pt’s turn up & frontline staff don’t have them on the list.

@HelenBevan Maybe it’s the mental models that evolved which altered the choice of methodologies ? Dominance or single ontological sense making? Since updated from Pascal. Source here - cambridge.org/core/journals/…

A history of change: How approaches to change have evolved in alignment with prevailing mental models & dominant corporate functions of the time: 1960s: Operations focus: change processes were linear, planned & efficient. Leadership followed a top-down approach 1980s: Finance focus: Shift to service-oriented, more strategic stance. Change as a means of increasing capital allocation & share value 2000s: Internet/tech focus: change leaders as renegades who innovated, moved fast and broke things 2020s: Design focus: As the complexity & interconnectedness of challenges continue to grow, large-scale systemic transformation @marziaarico/designing-change-7beaa944b8a6" target="_blank" rel="nofollow noopener">medium.com/@marziaarico/d… By @marziaarico

@JohnWSeddon @JohnWSeddon couldn’t it also be the case that when the acuity, needs & goals of pt’s are matched with the right expertise, it’s entirely appropriate for the few to take up more clinical capacity, as they are performing at the top of their scope to deal with the variety?

Absolutely the big lever. In every service studied we find a small proportion of patients consuming a disproportionate amount of capacity - it's failure demand at scale.

@JohnWSeddon Mixed experience of the NHS today. Visited my local hospital, needed Radiology C. Only A and B listed on the hospital layout. Took me longer to find it than to have the scan. Staff as usual, were lovely. Then came home to this classic. WTAF? 👇🏻 1 of 2

@DrGW0 @alisonleary1 Productivity doesn’t feel right but I know what you mean on the sentiment. My MSc diss looked at cases of ‘whole system approaches’ in Wales = Staff trying to make a difference. The lit review touched on 70 k staff post pandemic leaving & with it tacit & idiographic knowledge

Pleased to share, my MSc is completed. My dissertation ‘Cases of Whole System approaches in NHS Wales & implications of Systems Thinking in Practice for #Improvement practitioners’ had recommendations aligned to the health & care quality standards. Thanks to all that contributed
