How do you protect muscle while losing weight? Protein intake and resistance training is the answer. Protein gives the raw material and strength training gives the body a reason to keep skeletal muscle. Without those two, the body tends to cut muscle.

Do people lose more skeletal muscle on Mounjaro/GLP-1s than in non-GLP-1 weight loss? Not clearly. Current evidence suggests the composition of weight lost on tirzepatide is broadly similar to standard weight loss: mostly fat, with some lean tissue.

In a standard well run weight loss journey, a rough rule of thumb is - 70–85% fat 10–20% lean tissue 5–15% water/glycogen fluctuation Not a law of nature, but a useful practical frame.

Very early 12.5 mg signal, but hard to ignore. The day 12.5 mg started, fasting glucose began dropping fast. Calorie intake followed. Weight then started responding. Interesting week!

12.5 mg Mounjaro is clearly more potent. Foul burps are back, hunger is muted, and there is a literal "knot in the stomach" feeling that removes the any urge to eat. My intake had drifted up to 2000 calories, has now crashed back to 1400.

Titrated to 12.5 mg Mounjaro today. Current NovoRapid use is already tiny, 0-2 units for breakfasts and 2-3 units at lunch. The experiment now is simple. Can 12.5 mg push the remaining bolus insulin requirement to zero? The next 4 weeks of data will decide.

The last year on Mounjaro has been so disciplined and ritual driven that I logged almost everything: every calorie, every minute walked, every mg/dL of glucose. I now have 360 days of metabolic data. A proper n=1 study. I do wonder if Lilly would be interested in it.

The math behind my move to 12.5 mg My data suggests every 1 mg increase in mounjaro reduced novorapid use by 1.35 units/day. So 10 → 12.5 mg = +2.5 mg. 1.35 × 2.5 = 3.375 units/day. My current novorapid use is about that. Hence the titration.

@muratkberber Temel sebep, 12,5 mg’ın kalan küçük NovoRapid dozlarını bırakmama yardım edip etmeyeceğini görmek. HbA1c zaten 5,4%, ama bazı öğünlerde hâlâ çok küçük bolus insülin kullanıyorum. Yani bu rastgele doz artırma değil, takip edilen bir titrasyon denemesi.

Lilly is pushing retatrutide to be classified as a biologic, not just a “drug.” FDA disagreed, and Lilly challenged it. A court has now told FDA to reconsider part of its reasoning. But why does @EliLillyandCo want retatrutide treated as a biologic, unlike tirzepatide?
