@DrJoGrady Where’s the money coming from? More pay = fewer staff given huge real-term cuts in tuition fees.
Maybe that’s for the best but it’s so dishonest not to acknowledge the trade-off

For World antibiotic awareness week I’d planned an UG practical about how gene expression is key to resistance phenotype. But I’ll be on strike because universities - where clever people work - can’t fathom how to pay their staff properly and provide decent pensions….

Personally, if I was a meropenem salesman and everyone instigated this approach, I’d be booking a new Porsche for delivery in mid 2025.

It’s the focus on 3GC use that worries me. Can’t do anything (according to my brain) except make the bacteriological soup more unpalatable.

Just published SR antibiotics to prevent pneumonia in ICU. Shows problems Bayesian presentations. Mortality RR 0.91, 95% credible interval 0.82–0.99. Borderline significance should leave uncertainty. Authors say 99.3% probability of benefit. Wrong message doi.org/10.1001/jama.2…

R rates are already non-trivial
R mechanisms diverse across multiple species
R genetically linked with virulence genes in some cases
R genetically linked with R to other critical abx.
Short term gain maybe, but long term pain likely.

But highly skeptical that giving all pt abx would actually reduce abx use in reality! Depends clearly on pt pop and prob of VAP. Additionally, giving *all* pt 3GC is *prob* worse than selected pt more abx I would guess - @MutantBug might have a view.
