@BJSM_BMJ Trial reflects practice as it should be done for most knee patients. Patients who are not happy with physical therapy outcomes can elect surgery. Note terms "standardized" and "individualized" describing physical therapy intervention.

@JoNijsBE @VUBrussel Contextual factors are not avoidable but controlled trials help account for contextual factors. Once evidence determines benefit beyond clinical interaction, comparison between competing clinical strategies are more relevant than continuous comparisons to placebo.
