Proud to share that our VITAL-HF trial is now out in press. VITAL-HF tested a remote, asynchronous digital strategy to help optimize guideline-directed medical therapy in patients with HFrEF who were not yet on maximally optimized treatment. In this multicenter randomized trial of 178 participants across 7 U.S. sites, the digital intervention improved the Heart Failure Collaboratory medication and KCMO score at 6 months compared with usual care, with consistent gains across the 4 foundational therapy classes and no signal for excess safety events. To me, the key message is that therapeutic inertia in heart failure is not just a knowledge problem. It is a workflow problem, a capacity problem, and often a between-visits problem. VITAL-HF suggests that digitally enabled, team-based care can help close those gaps by creating actionable opportunities to start, titrate, and sustain life-saving therapies outside the walls of the clinic. This is not the end of the story. We still need to understand implementation, scalability, access, and generalizability across broader real-world populations. But these findings are an encouraging step toward more proactive and effective heart failure care.
