2 siblings but completely different levels of care at different hospitals Despite presenting with more severe gastrointestinal symptoms, including chronic constipation with gaps of up to three weeks between bowel movements, faltering growth (31.0 kg at age 11), abdominal pain, faecal loading on imaging, and a clinical suspicion of Hirschsprung’s disease, a colonoscopy was abandoned mid-procedure at the descending colon due to poor bowel preparation. Crucially, no biopsies were taken from the terminal ileum, and no suction biopsy was conducted to evaluate for Hirschsprung’s disease. Although the endoscopy team documented “very poor mucosal visibility” and noted the procedure was abandoned, later clinical summaries inaccurately described the colonoscopy as “normal,” creating a misleading picture of diagnostic completion. No rescope was offered or planned, despite the fact that bowel prep could have been repeated, and objective evidence of slow transit constipation had already been confirmed via a colonic transit study In contrast, a sibling had symptoms come on much later and had similarly suboptimal bowel preparation However, the procedure was completed up to the terminal ileum, where visible ulceration and inflammation were observed at the ileocecal valve, and biopsies were appropriately taken from the ileum, caecum, and sigmoid colon.
