@CarloBellini @altcap This is spot-on. Calcium scores are like looking at rust to see if the car is broken. Young people die from soft plaque ruptures — invisible on calcium scoring. You want a CT angio if you’re <50 with risk. Don’t let a zero lull you into a false sense of security. @stevejang

I’m an MD who spent most of my clinical time in cardiology Many young people who die of heart disease will have a calcium score of zero As such it’s not a good risk stratifying test for the young for the following reasons A calcium score measures the amount of calcified plaque in the arteries. This is what we call hard plaque The type of plaque that kills most people is soft plaque The way plaque develops on an artery wall is: Normal wall -> soft plaque -> hard plaque (calcified) As such, calcium scores can be misleading especially in young people Because they measure calcified plaque only You could have material soft plaque and a calcium score of zero What kills many people suddenly is soft plaque rupture - this is when a bit of plaque breaks off for reasons we don't fully understand but believed to be related to inflammatory processes. The underlying artery wall is exposed and triggers a clotting path blocking the blood flow and potentially resulting in cardiac arrest So - you care about soft plaque. If you have it you want to treat it and try and stabilise it as much as possible So just doing a calcium score will not show you this It actually can lead to talse reassurance. The calcium deposits are related to age and time - so the group you talk about 35 yo onwards, will almost by definition have a zero calcium score because there hasn't been enough time to deposit calcium <4Oyo will nearly always be soft plaque 40-50 soft plaque might start to calcify So if you have any risks the test under 50 the test you want to get is a CT coronary angiogram which will reveal soft plaque which is much more useful than a calcium score in younger people (<50)
