Would strongly recommend that no one ever fly @AirFranceFR @airfrance. They downgraded our families trip for my wife’s 40th birthday from business, without explanation. Would be great if this could get some traction. Any help is greatly appreciated

@theliverdoc Sleep MD here…great post. Melatonin as a supplement is not regulated by FDA so we get patients taking 20mg +QHS. Soporific for some, but by in large useless for insomnia. Gold standard treatment is Cognitive Behavioral Therapy for Insomnia.

@DGlaucomflecken I think it depends on the student. No need for someone going into pathology to do more that a night or two of float IM or OB. Students going into fields that have overnights can do them during their Sub-Is and show rotations.

Kinda torn on this. I don’t think a night shift as a med student is necessary to learn how to practice medicine. However, I think it can be helpful as a way to foster compassion for your fellow physicians. Seeing the chief surgery resident fall asleep mid procedure note after operating all night left an impression on me. Maybe it’s a good idea to show a med student going into internal medicine or emergency med what surgery residents are dealing with and vice versa. That doesn’t mean a student has to work more than 14 straight hours. How about a night shift surrounded by days off? We need to understand each other’s work lives a little better, so we can be nicer, more understanding colleagues. Med school is the perfect time for that to happen before we all get lost in our own specialties.

@AnthemBCBS Oh I already had a P2P my “peer” told me my note was too long. So basically you have employed people who are making medical decisions affecting patients lives without reading physician notes.

So @AnthemBCBS denied an in lab psg for patient with HFrEF, EF 30%, BMI > 40 and bicarb of 33. Just a reminder that physicians who work for insurance have no concern for patients.

@DGlaucomflecken First off tavern style is better. But if you need to get Deep Dish, then Pequods. The sausage at Lou Malnatis is also very good. Lastly do yourself a favor and get an Italian beef from Mr. Beef.

If humans cut themselves off from all zeitgebers and follow their natural rhythm they would free run akin to Non24. Humans biological rhythm is around 24.15 hours give or take. This is not the reason that most people have trouble sleeping. There is no biohacking to be done here.

SHOCKING 🚨 : In 1972, A French scientist Michel Siffre volunteered to lose his mind. He spent 180 days alone in a cave 440 feet underground with: - No clock. - No sun. - No human contact. What he discovered haunts scientists to this day:

Have been trying to be patient, but @UMichFootball is an embarrassment. There is no fight, the play calling is uninspiring, this is all on the coaches who do not deserve to be leading this program.
