Incidentally, keeping endomorphy <3 (for a guy) basically means keeping your sum of 6 skinfolds under 60mm.

@tomasrb Yes, indeed. At that point, instead of pushing higher statin dose, one could try other apoB-lowering medications that do not affect brain cholesterol synthesis: ezetimibe, bempedoic acid, PCSK9i

Better to ignore the colors and the "Normalized Value" (Sterol/TC in molar concentration). What matters most in an INDIVIDUAL patient is the absolute concentrations. In this case lathosterol is 45th %tile, desmosterol 20th %tile, sitosterol 55th %tile & campesterol 35th %tile. So, there is NORMAL synthesis & absorption. Only issue is if one is on a statin & an E4 allele is present - not wise to let desmosterol (a biomarker of brain cholesterol synthesis which correlates with cognition and AD) drop < 20th percentile. To reduce apoB a low dose statin + ezetimibe would be appropriate and if needed (depends on desirable goal) ultimately a PCSK9i

Does this indicate that liver clearance/ldl receptors are the primary issue and PCSK9 inhibitors would be a promising intervention?

"Hyper-response" to statins typically occurs in those who are hyper-synthesizers of cholesterol (check lathosterol & desmosterol levels before Rx) or those with hypo-absorption (sito- / campesterol). Tests are available @BostonHeartDX or DTC at @empowerDX @society_eas @docsanz

A strong case has been made for the addition of a strength/muscle-based event to our "Old man Olympics", so I propose we add... 50 consecutive push-ups. Going by ACSM's norms, this is actually tougher than the VO2max standard, with >39 consecutive push ups being a 95th percentile performance for a 50+ yo male, but our dude looks pretty buff. I don't think he'll have a problem with it 😊

Building Metabolic Fitness Metabolic fitness is the foundation of your Endurance Journey How do you get this done? Track your fitness with periodic lactate testing: ✅10 minute steps ✅Kick'r bike in erg mode Tips: 1⃣ Start too easy 2⃣ Size up the lancet (28 gauge for me) 3⃣ Spear the 3rd drop, don't touch skin THEN: 1⃣Find Your 1st Lactate Threshold (LT1) - the highest power/pace you can manage without lactate rising. One year ago, my lactate was rising immediately. My heart rate was ~60 beats below max. Now, it's rising ~40 beats below max. My easy zone covers 65% of my heart rate reserve. 2⃣ Train Easy - the humility to develop our low end is not helped by the constant discussion of which high intensity protocol to use. Focus on building General Capacity, first. 3⃣Whole Body Fitness - best in class metabolic fitness is whole body fitness. Flatten your lactate curve. Ideally by moving it down down, and to the right, in everything. 4⃣ Consistent Green Zone Volume - the faster we rack up 90+ minute sessions of easy volume, the faster our metabolic fitness will improve. Optimize for long-term volume with frequent 90+ minute sessions. The shift you see in the curve was achieved with ~14 hours per week of training. 700 annual hours is a ton compared to the average citizen. It's peanuts compared to an elite cyclist, swimmer or triathlete. Especially, if you adjust for the fact that my work rate is LOW. 5⃣ Eat Real Food - when we are focused on the low end of our curves, our heart rate is down and we can digest anything. There's no need to be pouring sugar into ourselves. Lollapalooza Effects All of the above, working together make the program less stressful on my body. As metabolic fitness improves, we have options: • Do More • Go Faster • Go Longer A year ago, everything made me tired. Today, I still get tired but it's from doing stuff I love: • My Wife's Birthday Swim (9400 LCM) • Mountain Rides • Big Day Training I haven't figured out the return to racing part, but it will come. Metabolic fitness expands our world and makes us feel better in our daily living. Best Part? It doesn't hurt.
