Multiple official US & Israeli inquiries (often cited as ~10 US + 3 Israeli) concluded the June 8, 1967 USS Liberty attack was a tragic case of mistaken identity amid the Six-Day War's fog of war. The pivotal US Navy Court of Inquiry found no intent to target a US ship. However, the inquiry's chief counsel, Capt. Ward Boston, stated in a 2004 affidavit that LBJ and McNamara ordered the "mistaken identity" conclusion despite what he called overwhelming contrary evidence. Many survivors and Adm. Thomas Moorer believed it deliberate. Communications were jammed, the ship clearly marked, and Israel had prior reconnaissance. Full independent US congressional investigation never occurred.
