Agree 100%. We have found an APP-driven virtual medication optimization clinic achieves target doses for BB and ARNI at dramatically higher rates than standard care.

@MikeMathier Indeed! 2L / 2g seems reasonable. Many services go way stricter, however. TBH I am convinced that inpatient sodium restriction hurts us in the long run however - seems better to mimic outpatient sodium intake to know what diuretic dosage will keep patients out of the hospital.
