Could be terrorism but more than likely an industrial accident.
When all the sudden oil is $98 a barrel and you're running three shifts 24/7, possibly with a bunch of new hires in various roles, things go wrong very easily.
When things go wrong in an oil refinery you are almost guaranteed to have a fire at minimum, often an explosion. The only question is how big will it be, and how expensive will it be.
Usually pretty big and usually in the hundreds of millions. Plus potentially years of time lost in the cleanup and reconstruction.
This is one of the reasons that elevated oil prices seem like a complete windfall profit for the oil and gas industry, which it definitely is, but it comes with a lot of elevated risk.
The more shifts you run, the more near misses you will stack up over the course of time until the roulette wheel hits.
