@Gabe__MD That is the hidden tax in a lot of workflows. Once alert noise gets high enough, the system starts stealing attention instead of supporting care. The fix is usually less about adding another tool and more about removing noise at the point of care. What alert would you cut first?

The safety system was on. It had been on for every patient, every order, every shift. That was the problem.
A 74-year-old woman with hypertension, heart failure, and Stage 3B CKD presents to her cardiologist with worsening edema. Medications: lisinopril, carvedilol, spironolactone 25mg daily, furosemide. Potassium running 5.1 to 5.3 for months — high but stable and monitored.
She also has a UTI. The cardiologist messages the PCP. The covering physician — someone who has never seen this patient — reviews the chart, notes the UTI, prescribes trimethoprim-sulfamethoxazole.
The EHR fires an alert: drug interaction, TMP-SMX and spironolactone, risk of hyperkalemia.
He clicks past it. He has clicked past 127 alerts today. Forty-one were drug interaction warnings. The system fires on aspirin and ibuprofen. On lisinopril and potassium supplements discontinued three years ago but never cleaned from the med list. On theoretical interactions that have never produced a documented adverse event in any patient at any dose.
The alert that matters — TMP-SMX plus spironolactone plus CKD is a potentially lethal combination — looks identical to the alert that does not matter. Same color. Same format. Same click-through. The system has trained him to treat every alert as noise.
Four days later she presents to the ED obtunded. Potassium 8.1. EKG shows sine wave. She codes. Calcium, insulin, bicarbonate, dialysis. She survives with anoxic brain injury from the arrest. She never returns home.
Now deploy AI.
Same patient. Same UTI. Same covering physician. The AI layer does not fire 127 identical alerts. It runs a contextual risk model: this patient, this combination, this renal function. TMP-SMX plus spironolactone plus GFR 34 plus baseline potassium 5.2 is not a theoretical interaction. It is a quantified high-probability adverse event.
The alert that fires is different. Flagged critical — distinct visual, distinct workflow, distinct escalation. It includes the last three potassium values and the calculated risk. It requires a free-text override justification, not a single click. It copies the prescribing physician and the cardiologist.
The covering physician pauses. Reads the potassium trend. Chooses nitrofurantoin. Checks the GFR — appropriate. The UTI resolves. Potassium stays at 5.2. She goes home.
AI did not override the physician. It separated the signal from 127 alerts' worth of noise.
