@NuelleSportsMD Looks like a trans tibial tunnel with suspension fixation on the femur. Absent the original indications and year? I could guess, but why? However many years later, there is medial compartment OA that is severe and depending on age (big range, and graft status) , Answer 2/2

Getting really tired of some “Democrats” acting like @DrBiden has no right to tell her side of the story or promote her own memoir. She stood by her husband and family, did the work of First Lady, kept teaching, showed up every day, and carried herself with humility and grace through more than most people could handle. She has more than earned the right to tell her story in her own words. I don’t know about you, but I’m looking forward to her book. I’ve got Dr. B’s back. Do you?

@drkeithsiau @hjluks Could be knee. not at all what we see with gout. swelling from a knee injury is usually diffuse/puffy worse later in day. Focal pain directly over the outer ankle or 5th metatarsal could suggest an actual foot/ankle injury such as 5th met stress rx/fx, ankle sprain

@hjluks It is not an exaggeration to say 85% (or more) of our patients would benefit from/need some form of physical or occupational therapy. For every patient that may need an operation I see 5 that don’t. Send. Them. To. PT.

Fun day .. “You’re my 5th opinion.” Knee injury These are usually a problem. Previous 4… -Only one exam -All repeated X-rays -2 MRIs. - One Special NY hospital often insists their MRIs are better and wants new ones 🤦‍♂️. The fourth doc started PT 🙌 🥳 4 surgical recommendations... but 3 different procedures. PT is working. Me: “Do you think you need an operation?” Them: No. I feel pretty good. Me again. “Cool… nice meeting you. Have a great weekend”. I wish these were rare or isolated cases.

@LenMacPT @agordon55 Re Achilles, the evidence is absolutely not the opposite. No blue check so my answers on X are short. Don’t know why you are so defensive. You clearly have a problem with your QT grafts. Why not try it?

Wish I had better results but also seems counterintuitive to put a muscle/tendon on stretch and isolate it with resisted extensions to prevent it from stretching out. We do the opposite for an Achilles and it’s still sometimes stretches out. What do you think is the mechanism and why do we have to be so specific? Does it apply to all quad tendon grafts or just full thickness dissection? What about partial thickness dissection? What about ones that use a bone dash tendon?

@doclsmack @LenMacPT Interesting… I haven’t thought of it this way before and it makes sense as something to consider and add in… if this approach really has a big impact for patients with QT grafts, the word doesn’t seem to have gotten out and the struggle with these patients is so massive.
