Let’s not forget, undiverted leak is more likely = recurrence or rectal cancer = not to be able to anastomotic leak to heal secondarily

One of the most important studies that should put an end to not to give defunctioning temporary ileostomy in the setting of ultra low colorectal / anal anastomosis. Our routine is any anastomosis one can reach with DRE ( More or less within 4 cm from anorectal ring) = divert

@FezaRemziMD Only shows 90 day lake rate, wel all know that longterm rates are equal and these late leaks are harder to treat. In stoma group more complications, higher ICU stay, longer hospital stay and higher readmissions.

Five years later, EssentiAL, the 1st of 3 major randomised trials expected this year, has shown significant benefit for icg in rectal resection in reducing anastomotic leaks by c 40% journals.lww.com/annalsofsurger… 11.8% down to 7.6% for all leaks & 8.2% to 4.7% for symptomatic leaks!
