@ProscanMRI Doesn’t even matter… if you press something hard enough into the tissues and tape it down it will distort the soft tissue and create artifact even without susceptibility of its own

What you are missing is that patients in your office demanding that something be done. And although on a macro basis they may not provide benefit there is a lot of benefit for a lot of patients and so it is something easy to try. Is it perfect evidence-based? No, but the reality is private practice medicine is a customer service business as well. Clinic is very hard. Good luck spending 25 minutes with every patient explaining to them that this shot doesn't work when it worked for their neighbor

Call me simple, but I’ve never fully understood the logic of chasing short-term results. For example, using steroid injections in an arthritic joint when the patient already knows they’ll eventually need a replacement. I can see the value if there’s a specific short-term goal like getting through a big event or an active summer but I doubt that’s the case in most scenarios. If me, I wouldn’t want steroids injected into my body I’d rather just have the joint replacement and move on with life. What am I missing? How often do you actually see a patient become asymptomatic after a steroid injection and enjoy years of relief? From what I’ve seen, the literature shows worse outcomes in patients who undergo multiple injections before eventually having arthroplasty. On top of that, steroids can harm bone health and have other neg physiologic effects. It feels like I’m missing something important here.

@ProscanMRI We don't generally either. A little experimentation here. I'm not sure it's necessary and I'm not aware of literature proving we need to do this however I didn't hate it.

@northwoods1980 @ProscanMRI We don’t. Patient positioning to pronounce specific anatomy is important ( like external rotation of arm for shoulder joint ) but a comfortable patient who can keep still is more important

Update - L radicular leg pain & foot drop almost resolved Unfortunately symptoms evolved to become bilateral gluteal, pelvic & groin - 2 weeks ago When the pain got to 9-10/10 & I couldn't sleep / walk, I decided to forego watching the Hungarian Grand Prix & head to Urgent Care at PGH instead - raised CRP & neutrophilia MRI on Sunday at Fortius (amazing service) - raging bacterial disciitis & epidural abscess Don't tell anyone but it's affecting the psoas so I can't SLR 😆 So now in hospital awaiting PICC line and 2-3 months antibiotics 😕❎

Update for those interested! 16 days post discectomy Minimal pain - low grade 2/10 neural ache buttock and fibula head / shin Worse at night and morning but variable Numbness is more annoying! Tib ant power recovering Doing simple upper body weights typically seated or cable Iso rehab for quad, hamstrings and tib ant Stationary bike & elliptical trainer - easy steady state I’m behaving and listening to my brilliant physio Chris Whittle @puresportsmed

My wife had to go private for MRI for a potential bone tumor because it would take 9 months otherwise. Her doctor admits that she herself is being screened for BRAIN CANCER and is waiting over 16 months for a CT scan! A doctor! My wife is horrified and asks why on earth she doesn’t just pay for one. “I will never pay for private healthcare out of principle” She’s literally risking death for ideology. I told my wife it’s time for a new doctor. One with a brain.

Our 'free' healthcare system in Ontario, just this week, from our family's experience: - son-in-law with likely torn ACL is told it'll be weeks for an MRI. Forced to go to Quebec and pay $900 to get one within the week. - daughter-in-law with intermittent tonsillitis refused tonsillectomy because it's not an emergency. Desperate, she will now fly out of province and pay thousands to get it done privately - son referred to imaging clinic for a concerning condition. Waits two months for the call. Calls the clinic booker who locates his file. Booker eventually agrees that, yes, they should probably book him in for an appointment in a couple of months. - friend of family with a severe hereditary susceptibility to breast cancer is given an imaging requisition and told to get it done as a high priority. Imaging clinic: no, you're not a high priority. This, we would do in one or two years. I don't remember it ever being this broken before.
