There’s a thread on Reddit’s r/emergencymedicine questioning my work in the gut–brain space. I understand why. Medicine is trained to be cautious about commercialization. It should be. We should question motives. We should protect patients. That skepticism is healthy. But here’s what I can’t accept anymore: The quiet resignation that follows “your labs are normal.” I’ve practiced emergency medicine my entire career. I still do. I’ve led. I’ve published. I believe deeply in this profession. And I’ve also watched patient after patient walk out with persistent nausea, pain, fatigue, bloating — told everything looks fine. Some of those patients are people I love. When the workup is normal but the suffering is not, we have two choices: We can defend the boundaries of what we already know. Or we can build better tools. The gut–brain axis is not fringe. It’s underdeveloped. And underdeveloped fields don’t advance by staying comfortable. Yes, I’m building. Yes, I’m commercializing ideas. Yes, I’m willing to take scrutiny. Because the greater risk to patients isn’t that someone tries something new. The greater risk is a system that decides “normal” is good enough. Progress in medicine has never come from protecting the status quo. It has come from physicians willing to be questioned while they do the work. If you disagree with my approach, that’s fair. Debate it. Improve it. Test it. But patients living in that gap deserve more than a shrug. And I’m not willing to stop at the shrug anymore.
