SP enables a lot of creativity which IMO is the best part about surgery.. innovative cases: 1. Intravesical diverticulectomy +/- rasp 2. Intravesical distal ureteral reimplant 3. No incision stoma site RCIC @RoboticsUrology 4. Retro LAA ureteral reimplant in hostile abdomens @urorogerio has example 5. Transvesical Urethrovesical anastomosis breakdown repairs 6. Transvaginal ureteral reimplant 7. Modified template retro rplnd left or right side 8. Retro nephropexy I’m sure there’s tons of other ideas @SimplyUroMD

All can check out what @RoboticsUrology taught me and what we do with SP. This is not a minor improvement like those who don’t adopt say (similar to robotics argument) putting aside obvious benefits when applicable in cases like RALP (no intraabdominal violation - virtually every patient regardless of previous surgery is candidate for surgery, near 0 if not 0 risk of ANY intra-abdominal comps). Even more, As @asajimd and @urorogerio have said, When one understands sp, patient centered direct site surgery allows for more pts to be ideal candidates, reduced unnecessary tissue violation, less anesthesia requirements leading to better postop recovery course. As @RoboticsUrology says, “there is a laundry list of complications that we no longer need to discuss”

When you fully leverage the range of approaches SP enables, it’s not incremental-it’s a fundamentally different,and in many ways revolutionary,shift.

Interesting data from #EAU26 2026 comparing BST vs RC. Higher recurrence, cost and anxiety with BST deserve attention. Before committing to BST, refer patients to high-volume surgeons—We routinely perform robotic RC (DV5/SP) in ~3–4 hrs with 0 open conversion, near 0 take back.

#EAU26 Does bladder preservation really improve QoL? Dr. Gessner reviews the CISTO study (n=570) BST vs RC in recurrent high-grade NMIBC. 🔹 Physical function at 12 mo: similar between BST and RC 🔹 RC: better emotional function, ↓ anxiety/depression, ↓ financial burden 🔹 BST: better bowel & sexual outcomes 📊 Oncological outcomes: • CSS similar (99% BST vs 96% RC) • BST → higher recurrence rates • RC → higher perioperative toxicity (90-day mortality ~2.5%) @uroweb @UroToday
