🚨 Aptevo Therapeutics $APVO Reports 90% Remission* in AML Trial In its RAINIER Phase 1b/2 trial, $APVO reported 9 out of 10 patients achieved remission when treated with mipletamig + venetoclax + azacitidine—a 90% remission rate, outperforming the ~66% rate typically seen with the doublet (venetoclax + azacitidine) alone. The results of today's case discussion are as follows. I. Cytomorphologic diagnosis. 1. ANLL-M5 2. Immunophenotyping, cytogenetics and molecular biology are recommended. II. Immunophenotyping. Acute myeloid leukemia with mononuclear differentiation. III. Cytogenetics. Conventional chromosomes show complex karyotype. fISH not sent. Molecular Biology. 56 myeloid leukemia genes were negative. 63 AML/MPN etc., positive for ASXL1, SRSF2, ZRSR2 and TET2. Combining the above findings, the patient was provisionally AML-MR according to, 2022 5th edition. Today's case discussion is the same as yesterday's; what the composite diagnosis is is not the focus of today's case discussion; the focus is on what kind of cellular morphology is likely to be mononuclear-like cells. Mononuclear lineage features. Larger cytosol, abundant cytoplasmic mass, cytoplasm is gray-blue, translucent, hairy glass feeling, young single visible fine purplish-red granules, granules are numerous and uncountable, nucleus is large, more irregular, and obviously depressed and twisted. Nuclear chromatin fine or coarse reticular structure, the original single generally no particles, nucleoli large and obvious, like bull's eye. If the cytosol medium size, cytoplasmic mass less, light blue, transparent, no particles or few particles, nuclear large, only depression, nuclear chromatin is fine, fine sand-like, nucleolus more, small, clear, is the granulocyte primitive cells, primitive granulocytes can also be cytosolic depression. Therefore, either the nucleus is a little depressed or irregular is mononuclear lineage.

Today's case is discussed below: Patient, Yang Moumou, male, 70 years old. The patient developed left arm pain more than 10 days ago without obvious triggers, aggravated by activity, and did not pay attention to it. 1 week ago, he developed generalized pain, bilateral rib pain obviously, accompanied by low-grade fever, up to 38.5 ℃, without cough and sputum. He was admitted to our hospital. Admission blood routine biochemistry report is as follows. During the course of the disease, the patient had poor appetite and sleep, and there was no significant change in weight recently. He was healthy in general and had a history of cataract surgery. Physical examination: clear consciousness, no sick appearance, no rash on the skin and mucous membranes, no subcutaneous hemorrhage. There were no enlarged superficial lymph nodes throughout the body. Liver and spleen were not palpable under the ribs, and there was no swelling of both lower limbs.
