@VigilantFox False. All goes back to hormones and lifestyle. Often times it’s the hormones which starts the cycle. If you don’t fix the hormones, then yes the cycle will repeat.

🚨I have bad news everyone 🚨 After 9 years eating ketogenic, no processed carbs, no sugar, very little fruit but plenty of steak and eggs, I regret to inform you about my most recent heart scan and the ultrasounds of my arteries. My most recent calcium scan was zero, and my carotids and femoral arteries are completely free of any plaque 😮 Why is this bad? Somewhere, on some corner of the earth there is a vegan who will be suffering to see me thrive eating animals.

@AnilMakam @nickmmark But why would we suggest orthopedic surgery without symptoms? That’s not on the MRI, that’s on us. And if we did more preventive imaging we would realize what was important and what is not.

Have bumped up my baseline to 20k/steps/day with spikes up to 35k every few days. ~70 miles/week or so. I originally did this to bump up my base, but I’ve noticed some other interesting changes. - Time compression: it doesn’t feel like I’m spending more time on walking. Time perception on walks has changed. It doesn’t feel long, doesn’t feel short. The only way I’m able to gauge it is where I am on my walk. Otherwise, I’d have no idea. - Noticing movement: I am hyper aware of my movement. I feel moving through space a lot more. I notice small differences in gait, or a muscle contracting differently on one side. This didn’t happen the same way at lower volume. - Mental noise: drops to essentially zero. I don’t have background chatter. I just put on some music, start moving, and it’s a lot more like I’m inhabiting a familiar space rather than training. I like it a lot.

What do you think about this patient’s health? 300 lbs. High blood pressure controlled with meds. Feels “fine.” Labs: Total cholesterol: 139 LDL: 81 Most doctors would nod and move on. Anything concerning so far? Let’s add more: Glucose: 89 A1C: 5.3 Kidney and liver function: normal Now it really looks good, right? If you stopped here, you’d call him “stable.” Maybe even “doing well.” But we’re not done. HDL: 23 Triglycerides: 293 That changes things. Low HDL. High triglycerides. Now we’re seeing smoke. So what would you check next? INSULIN. Fasting insulin: 44.6 That’s not slightly elevated. That’s metabolic alarm bells. Here’s the scary part: If he saw his PCP, insulin wouldn’t even be ordered. He’d get a reassuring smile. “Everything looks fine. See you next year.” Meanwhile, under the surface: - Severe insulin resistance. - Accelerated cardiovascular risk. - Future diabetes loading quietly. Normal glucose does NOT mean normal metabolism. A normal A1C does NOT mean you’re healthy. Insulin resistance hides in plain sight. The question is: Are you measuring the right thing?

Most in the same breath tell me that they want results, but can’t stay consistent. Brains love dopamine now, not 90 days from now. - Design the win (“prep clothes night before”) - Vague 'why' fades fast. Tie it to a deep why (kids, freedom, pride) - Start stupid small (build momentum) - Track direction, not perfection (Perfectionism kills progress ('If I miss one day, game over') Repeat/Build on this and then you will see the results.
