Case By Dr Prabhat Agarwal,Prof of Med, Agra 26-year-old male with CKD, on dapagliflozin 10 mg, presented on routine follow-up with hemoglobin of 18.3 g/dL. His baseline hemoglobin before starting dapagliflozin, 6 months earlier, was 13.4 g/dL. He is a non-smoker. Dapagliflozin and Rising Hemoglobin: When Benefit Becomes a Red Flag SGLT2 inhibitors commonly increase hemoglobin and hematocrit, and this is now recognized as a class effect, not a laboratory oddity. This rise is not merely due to dehydration or hemoconcentration. Dapagliflozin also appears to stimulate true erythropoiesis through changes in renal oxygen handling, erythropoietin signalling, and iron regulation. In CKD, dapagliflozin has actually been shown to prevent or correct anemia, which is usually considered a favorable cardiorenal signal. However, an increase from Hb 13.4 to 18.3 g/dL in 6 months is too large to be casually labelled “expected.” This should be treated as erythrocytosis until proven otherwise. So the key clinical question is not “Can dapagliflozin raise Hb?” It can. The real question is: “Is dapagliflozin the whole story?” In this case, dapagliflozin is a very plausible contributor, but the magnitude of rise suggests either: (a) amplified drug effect, (b) volume contraction, or (c) an additional secondary cause of erythrocytosis. First practical step: repeat CBC after ensuring proper hydration. A dry patient can look hematologically dramatic for no good reason. If Hb remains high, think beyond the drug. Important causes to actively screen for include: sleep apnea, chronic hypoxia, testosterone/anabolic steroid use, smoking/vaping, dehydration, and occult renal hypoxic/EPO-driven states. In a 26-year-old male, one must specifically ask about gym hormones / testosterone / anabolic agents. This is one of the most commonly missed causes of secondary erythrocytosis. Polycythemia vera is less likely, but not exempt from the differential. Persistent high Hb/hematocrit, especially if unexplained, should prompt consideration of JAK2 mutation testing and serum EPO. A useful bedside principle: “Drug-induced erythrocytosis is a diagnosis of probability — not permission to stop thinking.” Published CKD case reports now confirm that dapagliflozin-related erythrocytosis does occur, and in some cases Hb normalizes after stopping the drug. Therefore, if Hb remains persistently high or hematocrit crosses the concerning range, a temporary withdrawal of dapagliflozin can be both diagnostic and therapeutic. The current evidence is somewhat reassuring in that SGLT2-associated erythrocytosis has not consistently translated into major thrombotic risk, but caution is still warranted in extreme elevations or high-risk patients. Bottom line: In this patient, the rise in Hb is very likely related to dapagliflozin, but too marked to ignore. The right approach is repeat, rehydrate, reassess, and rule out secondary causes before calling it benign. Suggested Clinical Work-up Repeat CBC + hematocrit Ensure good hydration Pulse oximetry Ask for snoring / OSA Ask for testosterone / anabolic use Consider serum EPO Consider JAK2 V617F Review for thrombotic symptoms Key Point “Dapagliflozin can raise hemoglobin — but when it rises too much, the diagnosis should not stop at the prescription.”
