Sending a letter by second class post and saying that you will discharge the patient if you don't hear from them within 7 days is just cutting waiting lists by stealth. It's not ok and it makes my blood boil

Has anyone else been offered an uplift this year for their local commissioned services? Just found out ours is 0.6% (you read that right, zero point six) 🤯 This was only offered after complaints about a lack of uplift for 6 years

Wanted to clarify something about a patient. Phoned the GP Within 90s of calling, the GP was telling me he knew the patient for the last 30 yrs, the full pmh, baseline exam and the patients hobbies too. When primary care are allowed to be primary care, they have no equal.

GP Gavin Shields, "We have far too many patients struggling to get GP appoints.. It costs more money in the long term" "We would like to employ more GPs and we know we have significant number of GPs unemployed, unable to find work" Victoria Derbyshire, "So employ more" Gavin Shields, "We can't because the core funding that goes into our practcies is not enough, and it has fallen in real terms.. And in England the contract says we are not allowed to employ GPs" Victoria Derbyshire, "Why not?" Gavin Shields, "We are not allowed" Victoria Derbyshire, "Maria Caulfield, what's the rationale for that? Why can't GPs practices employ more GPs?" Maria Caulfield, "The are roles for outher healthcare professionals.. That was agreed with GPs" Victoria Derbyshire, "So you can employ other people to help?" Gavin Shields, "We can't employ practice nurses"

Please read this tweet and thread from a GP about how the Tories have ring fenced money which can't be spent on more GPs It's insane and dangerous 😡 ⏬️ I had no idea. Did you? I might cover this on my @LBC show tomorrow @liambarker

My respect for GPs quadrupled when I worked as FY2 on GP Placement. This is what I witnessed. - GPs had excellent knowledge base which was wider than any specialist and they were keeping that up to date with ever changing guidelines and protocols. - GPs had a constant flow of patients, requests from patients, investigations follow ups and paperwork from hospital. Even in their lunch breaks they were sorting out letters from the hospitals. - Due to delays in the hospital appointments and time between the specialist clinics reviews, GPs were often sorting out the patients in between by trying to keep the symptoms under control and monitoring for any red flags. - GPs had a real presence in the local community and the local care homes, elderly patient and vulnerable group of patients saw them as problem solvers. GPs were getting to the patients who couldn’t come to the surgeries. - GPs were working with the ethos of keeping patients well in the community and trying to keep them out of hospital and secondary care as much as possible. - One urgent secondary care referral could delay GP surgery by 30-60 min, but yet they were managing that time very well and finishing their clinics without any cancelations. The dedication and commitment I saw in primary care was incredible and this experience was back in 2007-2008. Primary care is the front line of health care and investment, and attention should focus on them as making larger hospitals doesn’t always help the flow of patient if the primary care is struggling.

.@JimBethell claimed that PAs can replace GPs as “complex patients all get referred” In a clinic of 30 patients today, I’d class 26 as complex 1 needed urgent admission ALL other 25 complex patients I managed fully in 1° care without referral You replace us, you break 2° care

Recovering access? Practices in our area provided enough access for 1 in every 2 people to be seen in practice in one month. They sent and received 16500 texts a day Over 40% of contacts were seen same day, not in 24 hours, the exact same day the patient contacted their practice Practices received as many calls per day, in our small, mostly rural old ccg area, as the whole of the ambulance service for an entire NHS REGION 70% of all appointments were delivered online More access than ever before, with zero deficit and improving efficiencies. Maybe, rather than imply failure - praise general practice for doing what no other part of the NHS has, rather than strip away funding through sub inflation rises, give the people stemming the rising tide of acute activity hope of a future that includes them. Maybe LMCs around the country should start hosting webinars about how ICBs can do more to be system leaders, or how NHSE could do a better job of contract negotiation...

As a Consultant I find it disgusting how General Practice is treated by the media. It has been an amazing part of the NHS since it’s inception, but the lack of understanding of what GPs actually do is appalling. Funding cuts are destroying GPs and the nation will sorely regret it

Seeing the same GP reduces your risk of an emergency admission to hospital or of needing to use the out of hours service by 30% and astonishingly reduces the risk of dying in the following 12 months by 25% And knowing your patients makes being a GP a wonderful job. @rcgp
