It is a spectrum and many with higher SVI are in a low flow state. 2- A valve area of 1 cm2 usually does not translate into a MG of 40. Usually 0.8. These are some explanations for the discrepancy when measurements are verified.

Would imagine that if someone has true severe AS, with sufficient stroke volume (SVI > 35 ml/m2), the mean gradient should be also in severe range (> 40 mmHg). Though, I do see this discordances in real life as well.
