Compromise on the standard of care: The figures chosen are reckless and arbitrary. They force doctors to cut corners to fit their management plan within the SHA's financial limits. Essentially, it would be correct to say that SHA is practicing medicine without a license. 2. The cost-shifting fallacy: SHA was purportedly designed to reduce out-of-pocket expenditures. This policy does the exact opposite—by shifting the cost of care from a multi-billion-dollar fund to poor patients' pockets at their most vulnerable moment. 3. Professional autonomy and violation of ethics: If a doctor knows a patient needs a battery of investigations, especially in specialist clinics, but only has 4k at their disposal, the result will, without doubt, be substandard medical treatment. SHA must revert to the comprehensive model that puts limits on general outpatient lump sum and not per-visit caps. To better understand what we are telling them, SHA should urgently audit how many patients walk away from the lab or pharmacy because their coverage only allows consultations. This change of contractual terms is unilateral and may amount to a breach of contract. A good healthcare financing system should not operate like a game of musical chairs, shifting goalposts and changing critical policy rules in an arbitrary trial-and-error manner.
