@BCBSTX I am seriously concerned by the mess-up and leaving me and my family without active insurance for almost whole of the month. I dutifully signup on marketplace in Dec, made payments using the link recvd, the amt gets taken from my account and I even get a set of cards. And then told that payment went to last year's plans and so my plan (and therefore the cards) are not active. And now it is 14 days of regular follow-up but still no resolution. The customer advocates have been extremely empathetic but cannot resolve without backend support. And everytime i talk, i get new ticket with new SLO being another 7-14 business days. I am completely clueless how to get this resolved fast. Any help from BCBSTX to get this resolved will be appreciated.

Appeal Letter – Medical Necessity for Inpatient Hospitalization Patient: Robert Stephens Admission Date: 12/30/2025 Facility: Houston Methodist The Woodlands Physician: Zain Pan, MD Diagnosis Codes: •Acute on Chronic Systolic Heart Failure (EF 20–25%) •Acute Kidney Injury on CKD Stage 3 •Pulmonary Edema •Multifocal Pneumonia •Hypoxemic Respiratory Failure •Non-Ischemic Cardiomyopathy •Hypertension •Transaminitis •Ventricular Arrhythmias (NSVT) ⸻ To Whom It May Concern: I am formally appealing the denial of inpatient hospitalization for Robert Stephens on 12/30/2025, based on the assertion that his condition did not meet medical necessity criteria. This determination is not supported by the clinical evidence documented during his hospitalization. Mr. Stephens presented with life-threatening, multi-system acute decompensation requiring continuous monitoring, IV therapy, and specialist management. The following objective findings confirm the necessity of inpatient care: ⸻ 1. Severe Cardiac Decompensation Echocardiogram Findings: •Ejection Fraction: 20–25% •Severe global hypokinesis •Severely dilated left and right atria •Moderate to severe mitral regurgitation •Grade III (restrictive) diastolic dysfunction These findings reflect advanced systolic heart failure with high risk of sudden cardiac events. Clinical Course: •Acute on chronic systolic CHF exacerbation •NT-proBNP elevated to 1210, later improving to 583 with IV diuresis •Required IV furosemide and oxygen •Non-sustained ventricular tachycardia (NSVT) requiring telemetry •AICD present and interrogation planned This level of cardiac instability cannot be managed safely in an outpatient setting. ⸻ 2. Hypoxemic Respiratory Failure & Pulmonary Edema CT Chest & X-Ray Findings: •Diffuse ground-glass opacities •Interlobular septal thickening •Bilateral peribronchial infiltrates •Cardiomegaly •Pulmonary edema favored over infection Clinical Management: •Required supplemental oxygen •Diagnosed with hypoxemic respiratory failure •IV diuresis initiated •Pulmonology consulted These findings represent acute respiratory compromise secondary to heart failure — a clear inpatient indication. ⸻ 3. Acute Kidney Injury on Chronic Kidney Disease (CKD Stage 3) Laboratory Evidence: •Creatinine: 2.06 → 1.70 → 1.43 mg/dL •BUN: 28 → 27 → 23 mg/dL •eGFR: 51 (abnormal) Nephrology Assessment: •AKI on CKD Stage 3 •Likely due to CHF and inflammation •ARB therapy held due to renal dysfunction •Strict intake/output monitoring required Renal instability in the setting of heart failure requires inpatient monitoring. ⸻ 4. Suspected Multifocal Pneumonia Supporting Data: •CT imaging: Patchy bilateral consolidations •Procalcitonin: Elevated •Empiric IV antibiotics (ceftriaxone + azithromycin) •Pulmonary consultation •Blood cultures negative Although pulmonary edema was favored, antibiotic therapy was medically indicated due to imaging and lab findings. ⸻ 5. Significant Fluid Overload Intake/Output: •Net negative –720 mL •Lost 16 pounds since admission This reflects significant volume overload, requiring IV diuresis and close monitoring. ⸻ 6. Liver Injury from Cardiac Congestion Labs: •ALT: 83 → 73 → 62 •AST: 52 → 59 → 41 •Albumin: 2.1 g/dL (low) •Total Protein: 5.5 g/dL (low) Diagnosed as congestive hepatopathy secondary to CHF — another marker of systemic decompensation. ⸻ 7. Need for Multidisciplinary Inpatient Care During this admission, he required: •Cardiology •Pulmonology •Nephrology •GI •Telemetry monitoring •IV diuresis •IV antibiotics •Oxygen therapy •Electrolyte replacement •Strict intake/output tracking This level of care cannot be provided safely in an outpatient or observation.
