Very soon, we will be assessing PCI results with non-invasive imaging. This case shows a left main crossover PCI to the LAD, where our main question was the result at the ostium of the circumflex. #CCTA will rapidly become an important tool not only for planning PCI, but also for evaluating its final result. @crfheart @PCRonline @SCAI @Heartflow @ACCinTouch @JACCJournals @ESC_Journals

National investigator meeting today for the SOS-AMI Trial—a trial that is as fascinating as it is challenging from both a conceptual and analytical standpoint. Patients are randomized before any event occurs and trained to self-inject the study drug if they believe they are experiencing an AMI. When an event actually happens, the comparison between selatogrel and placebo remains randomized and is evaluated using a 6-level ordinal clinical scale. The study needs about 5,000 self-injections, with roughly 1,000 expected true events. To reach this, nearly 15,000 patients have already been randomized. So far, however, the rate of self-injection is lower than expected, meaning additional enrollment will likely be needed and the completion date may shift. Beyond evaluating a novel self-administered P2Y₁₂ inhibitor, this study will also teach us a great deal about the feasibility—and inherent uncertainty—of trial designs that place patients’ decisions at the center.

Great contribution by @ColletCarlos @BernardBruyne @CoreAalst demonstrating intracoronary papaverine as a safe and effective option for achieving maximal steady-state hyperemia (~1.2% risk of ventricular arrhythmias), aligned with recent @AHAScience suggested practices from @SripalBangalore and @wfearonmd. Nearly 40 years ago, Bob Wilson @UMN_DCVM described papaverine as the “ideal” coronary vasodilator and also reported ventricular dysrhythmias, particularly in women. In this contemporary cohort, female sex remained the only independent predictor of arrhythmias. Should we consider weight-adjusted or lower dosing in smaller patients, particularly women?
