The most important change IMO doesn’t appear in the visual abstract. It’s the acknowledgment that all CKD patients should be screened for iron deficiency. Iron deficiency is more common than anemia in CKD. I recommend annual checks of TSAT, ferritin, serum iron, and CBC.

Does mechanical thrombectomy actually improve meaningful outcomes in intermediate-risk PE? 🤔 In the STORM-PE randomized trial, thrombectomy improved RV/LV ratios at 48 hours—but clinical outcomes were essentially identical to anticoagulation alone. Mortality, ICU and hospital length of stay, and clinical deterioration were similar, raising an important question: are we chasing imaging surrogates instead of patient-centered outcomes? For now, anticoagulation alone remains a perfectly appropriate choice for many intermediate-risk patients. How will this study change your current practice? @WilliamAird4 In our experience women are happy and welcome talking about periods and the recognition of heavy periods - to discuss how much fluid a tampon hold vs Maxi etc - this is normal for women and not a taboo : it must be a conversation and not hidden

Poll result: most respondents found “Mother Nature” explanations for menstrual iron deficiency off-putting. I’ve used that phrase myself. We rarely use comparable language when discussing male physiology, which may explain the reaction. Language patterns count.

Previous guidelines suggest that pregnant women with sickle cell disease should be prescribed low-dose aspirin to reduce the preeclampsia and IUGR risk. Our new trial indicates this practice may be harmful. Very pleased to have been part of this @cctris_ trial.
