@MicieliA_MD The annoying part of CTP in this new era of large core thrombectomy. I’m taking this patient to the lab after a thorough discussion with the family. Indifferent on the TNK based on early changes on CT, could go either way.

Patient presents ~1 hr from witnessed onset aphasia + R sided weakness. NIHSS >20.
CT/CTP 👇. L M1 occlusion. Reduced extent of collaterals on single phase CTA.
Radiology reports a large core, ED questions the utility of treatment based on CTP.
What do you do?

@AdiG1993 I’m not CTA for all, just sharing a case relevant to the original tweet. Big believer of speaking to a patient and looking at the non con (dense basilar in this case) when treating sudden neurologic complaint. Nice localization!

You have a hemisensory loss.
This localizes to the contralateral VPL nucleus of thalamus or the subcortical somatosensory fibers beyond.
One does not need a CT angio to DIAGNOSE this per se.
Only for therapeutic reasons.
