@FSQMD2 @STateshimaMD You’re asking to IVUS every single vessel to look for an aneurysm? Look how small of a vessel this dissecting aneurysm arises from. There is a role for IVUS in neuro IR, don’t think this is one of them.

A delayed AICA/PICA dissection detected on Day 7 after a negative Day 0 study. In high-grade or non-perimesencephalic SAH, repeat DSA at 5–7 days is mandatory—posterior circulation dissections can declare themselves late and rebleed if missed.

@AdiG1993 I’m not CTA for all, just sharing a case relevant to the original tweet. Big believer of speaking to a patient and looking at the non con (dense basilar in this case) when treating sudden neurologic complaint. Nice localization!
