Terrific thread on a fascinating study with potential high clinical impact for those who do hospital-based care. Thank you, @robertpdickson! Is depletion of anaerobes the mechanism of cefepime's superiority over pip-tazo? (Cefepime has anaerobic activity, albeit not as broad)
Robert Dickson@robertpdickson

Most patients with sepsis don't need anti-anaerobic antibiotics. But most get them anyway. Why not, what's the harm? I worry the harm is considerable. Our new study in @JAMAInternalMed: using a 15-month pip-tazo shortage to answer this question. 1/n jamanetwork.com/journals/jamai…

Humbled to announce that I have been selected to serve on RFK Jr’s presidential campaign team as a member of the American Safe and Secure Vaccine Advisory Committee (ASSVAC)! Our committee certainly has folks with unorthodox viewpoints, so can’t wait to learn!

Alright #IDTwitter, what would you recommend in the situation below? Source control unattainable, infected graft so hoping for short-term acute treatment and long-term suppression. NDM positive Providencia rettgeri

@sigal_md @PaulSaxMD Yes, agree w you and Jake. that’s the problem — it’s hard to give clear guidance to teams on when to treat (and empirically cover for a history of previous coryne) other than ‘vibes.’ But it does feel like we are way over-treating

@BrianTanChan @PaulSaxMD 🤷🏻‍♀️ like other colonizers in copd/other chronic/nosocomial lung disease hard to know when just selected by our resp abx, a marker of a bad situation, or its cause. Agree it’s often just there for the ride., but some report presumed lower tract infxn e.g wwwnc.cdc.gov/eid/article/28…

@BrianTanChan @JakeJohnsonMD Interesting question! I would, unless there's a contraindication. Basing this on the molecular studies of brain abscess that find a whole load of organisms that are never cultured. How do we know they're all susceptible to beta lactams? An early example: doi.org/10.1093/cid/ci…

Brain abscess with strep, odontogenic source. Healthy young man otherwise. Am I obligated to treat with flagyl for anaerobes too? Shouldn’t the anaerobes all be mouth flora? #IDTwitter @PaulSaxMD @JakeJohnsonMD

Listening to a lot of 90s records with my girls recently. An album that’s better than I remember is Cracker, Kerosene Hat. An album that’s worse than I remember is Soul Asylum, Let Your Dim Light Shine.
