@Charlie_S_Dai After curative tx, pts are Schrödinger’s cats: potentially cured or w/ invisible disease. MRD may let us “open the box,” test if tx escalation can convert MRD+ to MRD−, if this translates into curing more pts, & ultimately learn if early m+ detection helps/harms. We need trials.

Still have trouble reconciling the paradox that a therapy used in the adjuvant setting can save lives, but early exposure for M1 disease may not. Think we need to better understand this biology, and who really needs intensified surveillance/treatment. nejm.org/doi/10.1056/NE…

HER2 IHC scoring was fine-tuned for trastuzumab. ADCs, though, may require more sensitive assays. In this pilot study, we show that HER2 quantification (HS-HER2, RPPA, HER2DX) better predicts T-DXd efficacy than IHC and suggest a role for payload markers. nature.com/articles/s4169…

People who are worried about AI think it will destroy the economy. People who are excited about it largely make the same prediction. How could it actually be a net positive?

In a prospective cohort of >130,000 US adults followed up to 43 years, moderate intake of caffeinated #coffee (2–3 cups/day) or tea (1–2 cups/day) was associated with lower #dementia risk and modestly better cognitive performance. No significant association for decaf. ☕🧠

Delighted to present our real-world experience with T-DXd at @PeterMacCC with support from @LoiSher @BCTrialsANZ 44% required dose reduction + ~10% discontinued T-DXd. Toxicities occurred at rates higher than reported in trials, eg nausea (any grade 94.7%, ≥G3 9.3%)! #SABCS25
