In the world of modern medicine, the adage “to err is human” takes on a frightening reality when applied to genomics. As a cardiologist, I have seen firsthand how the advent of Whole Genome Sequencing (WGS) has revolutionized our ability to understand the genetic underpinnings of cardiovascular disease. However, with this immense power comes a significant responsibility: the need for absolute accuracy in the interpretation of WGS data. Imagine this scenario: A middle-aged patient, let’s call him John, comes into my clinic with a family history of hypertrophic cardiomyopathy (HCM), a condition known to cause sudden cardiac death. His WGS report, performed at a well-regarded lab, indicates a pathogenic variant in the MYH7 gene, a strong indicator of HCM. Based on this information, I recommend an implantable cardioverter-defibrillator (ICD) to prevent sudden death. But what if that WGS report was wrong? What if, in reality, John does not carry this variant, and the recommendation for an ICD, an invasive and life-altering procedure, was made based on an erroneous result? Alberta and Canada has been struggling with #CancerCare. The day when patient hear this word from their Doctor, their lives change. They start to think about their families and future plans they had for their loved ones. They struggle mentally and physically, waiting for possible good news from doctors/healthcare provider. Once they get over this hurdle (which is 2 month or longer “waitlist”), their next waitlist starts. Its to get Biopsy, confirming type of Cancer and see if they need any Genetic Tests for the Cancer. It helps with target therapy selections, but often this is not available in the public healthcare system. If its a slow growing cancer, ie prostate cancer, they could be waiting for 3-6 months for results to come back. Alberta doesn’t have capacity to do Cancer “DNA” and Pharmcogenomic tests in timely manner, and often have to send samples overseas that compromises privacy of Canadians. There is another waitlist, getting approvals for treatments. And, they can’t get until this Genomic evaluations are done. Sometimes, in breast cancer they do focused test for BRCA1/2 genes BUT almost never do complete WGS analysis to see if there could be other genes linked with cancer, and pharmcogenomic evaluations are done in timely manner. This leads to over or underdosing chemotherapy drugs, which sometimes leads to development of Heart Failure and other chronic conditions. This is preventable BUT system doesn’t want to offer these services to patients. Its the taxpayers, who should have access to best tests possible but somehow “some manager” decides that some lives are not worth saving and others may be, sitting far away from patients home. If patient survives these challenges, and cancer, there is no real system in place to follow them with Cell Free DNA tests to monitor for reoccurrence of cancers and PET/CT wait times can be upto 2 years. Its due to increase in demand with increase in aging population, lack of additional funding to meet demand, Outdated New Lab Testing Approval processes and expenses linked to Oncology are making access to Cancer care difficult for many patients. Many Canadians are struggling to get timely appointments with Oncologists, BUT even if they get to see the Doctor, they will struggle to access various imaging tests such as PET/CT and MRIs etc in time, And if they need radiation, they may have to go to Govt Funded visits outside the country. Let’s assume, Govts somehow get their act together and provide above access on the same day (less likely, but let’s assume they can), Canadians will not be able to get Genomics, Multi-Omics, Pharmacogenomic or Functional Cancer Assessment on time. There is NO significant investments in this area by Federal or Provincial Govts, if anything, they are sending DNA samples of Canadians overseas at astronomical costs. This increases risk for data breached and a sample of Canadian’s DNA stays overseas raising ethical and legal questions. One such case was Prostate Cancer Centre in Calgary. Their Metastatic Patients are struggling to access Advanced Genomic Testings, and after much of struggle, @BioAroInc stepped up and donated 100 tests for FREE (Donation upto $1 million). In the history of Canada, its the largest donation of its kind. It shows that you could have all the money, BUT when it comes to get the MultiOmics tests in Canada, you are helpless. We are lucky that we have Biotech company like #BioAro in Canada, but its upto govts to step up and provide funding for Canadians/Albertans to get State of The Art testing, cheaper and in timely manner. BioAro’s World Class software #PanOmiQ can do #WGS analysis on the same day vs #APL or other labs taking 6-8 weeks for most cases to get that report. I understand that I have bias towards BioAro, but what’s stopping Federal and Provincial Govts to set up same labs? If we could do it in Calgary, at a smaller budget, imagine what they could do with money power they have? #DiscoverGenomics

Its not that ASA is a problem, its prevalence of genetic mutations that makes people non responsive and not getting tested. Patients are using it, believing doctors (including myself), and are not aware. Anywhere 5-30% of patients are non responders to ASA. We, as Doctors know this, still are favouring ASA because there are no publicly funded Pharmacogenomic assessments available for Canadians. They should rather suffer, get ulcers and bleed without knowing if the drug they are using is helping them or not? I wouldn’t take ASA or any chronic use medicines without PGx assessments, and I would also suggest you to be the CEO of your own health. Know, if drugs are right for you and ONLY PGX evaluations can help. #BioAro
