@mani04 #3DEchoBerlin By the time GLS is reduced, there is replacement fibrosis by #WhyCMR & it’s irreversible (too late!) Use circumferential strain(SAX view #echofirst) which ⬆️ early in disease progression (ie hypertrophy) &only starts to ⬇️when get interstitial fibrosis

@argulian I think it is mid-ventricular HCM with apical aneurysm. I saw two similar cases recently. The wave (3) below mitral inflow E is the “suction of blood from the pouch into the chamber proximal to obstruction ”, wave 4 is the filling of pouch during isovolumetric contraction phase
