Young diabetic female, ant STEMI, taken to timely PPCI where she received a 2.5/38 stent in a totally occluded 4 mm LAD with TIMI3 flow, now 3 months post procedure, asymptomatic, normal LV, comes cause she wants you or me as her cardiologist. Poll in thread

My primary care doc bills my insurance $100 when he does an EKG in-office. He wanted another, I said could I go to my local hospital to get one? "Yes." I went by, they said would do it. I asked how much-- $1,200! I said no thank you. My doc did it. Time for uniform pricing in US.

@Cardio_experts @KardiologieHH Thank you dear Dr Edris, a few years ago I would have ad-hoc’d the Ramus and RCA with 2 stents. Now my understanding has changed a bit, if the CAD is causing the HF(which is very unlikely) its be indicated for revasc. Otherwise will Rx OMT 4 pillar HF and defer revasc.

I send my cardiomyopathy patients for cardiac MRI at one of the biggest academic centers in the country and they get date for CMR 2 months out, neither patient not doc waiting that long. Guidelines is one thing, readily available modality with outstanding reader is another.
