Thoracic epidural mystery. Tried different levels, LOR with hanging drop, could not advance catheter. Had this problem on multiple levels…Last attempt, LOR same as the ones before, advancement of catheter was not smooth, but TEA worked like a charm…any words of wisdom?🤷🏽♀️

Use the one that the clinician finds more necessary depending on the patients needs: i.e. peripartum cardiomyopathy and unsatisfactory response to phenylephrine would suggest to use norepinephrine

@runa123 Hi Runa, the most classical presentation is fetal heart rate abnormalities. Breakthrough Pain when it’s there should make your suspicion higher. Abruption might present with mild pain and your epidural might cover it. Look at the whole clinical picture in both scenarios

I have a question for anesthesiologists around the world involved in labor epidural analgesia(LEA).
When uterine rupture or placental abruption occurs to the parturient with LEA, does she complain of severe pain even if the extent of anesthesia is adequate? ＃OBAnes #MedTwitter
