Thoracic epidural mystery. Tried different levels, LOR with hanging drop, could not advance catheter. Had this problem on multiple levels…Last attempt, LOR same as the ones before, advancement of catheter was not smooth, but TEA worked like a charm…any words of wisdom?🤷🏽‍♀️

@MarcVandeVelde6 Agree with @MarcVandeVelde6 : 1) if you are resiting you might wet tap again 2) if resite top up dose unknown for emergent section 3) during resiting —>we had fetal Brady —>emergent section under GA

@runa123 Hi Runa, the most classical presentation is fetal heart rate abnormalities. Breakthrough Pain when it’s there should make your suspicion higher. Abruption might present with mild pain and your epidural might cover it. Look at the whole clinical picture in both scenarios

I have a question for anesthesiologists around the world involved in labor epidural analgesia(LEA). When uterine rupture or placental abruption occurs to the parturient with LEA, does she complain of severe pain even if the extent of anesthesia is adequate? #OBAnes #MedTwitter

How about preparing for the inevitable pharmacological sympathectomy due to propofol (or other anesthetic agents) with preemptive CV support (infusion +/- bolus of vasopressors and/or inotropes) as a message? @pwierusz @nei_scott @KhannaAshishCCM @atscommunity @JCVAonline

Peri-intubation #cardiovascular collapse in #ICU Propofol ➡️the most common induction agent (41%) ➡️associated with collapse (OR, 1.28 [1.05–1.57]). 👉Avoiding propofol may reduce cardiovascular collapse in the peri-intubation period in ICU. 🔗atsjournals.org/doi/epdf/10.11…
