In most public Irish hospitals that I have worked in over the years, applying to local hospital management for drug reimbursement from the hospital drugs budget had been an option in exceptional circumstances, or for rare cases. Typically, in these cases, an urgent, or at least quick funding decision is required. This is no longer possible in @HSELive hospitals. The capability for local reimbursement approval in rare cases has been formally revoked, ironically, at the same time that Regional Healthcare Areas have been established. Oncologists (and all other consultants) now instead have to complete a heap of paperwork, seek permission from their Regional Executive Officer, and the @CcoHse to have a funding decision made by HSE central management. This is a major change in policy, that will at best delay decision-making in often urgent situations. These strategic delays in drug funding decisions - if adhered to - will result in treatable conditions going untreated.

Thank you to @RTERadio1 for this discussion of cancer drug reimbursement in Ireland. Because private insurers and the public healthcare system (@HSELive) have different reimbursement criteria when it comes to newly @EMA_News-approved anti-cancer drugs, as things stand, cancer treatment will be increasingly different in public and private settings. Currently cancer patients in private hospitals have more treatment options. Not all @EMA_News approved cancer treatments should be reimbursed. The first concern should be effectiveness, the second unmet clinical need, and the third, price. These criteria should be assessed in public, in real-time. I agree that the nursing team at the NCCP have done a great job on this triage project. I know a lot of people at the @hseNCCP have put a lot of time and effort into this, and it is to be commended. To clarify, when the SOS hotline suggests that hospital admission is recommended, that admission in many HSE hospitals, particularly out of hours, will still be through the emergency department. This worthwhile scheme will hopefully reliably tell those that don’t need to attend ED - from a technical, medical perspective - not to go to the emergency department. In that circumstance, appropriate review by the nursing service will be arranged. Most days of the week, I see cancer patients in the emergency department who are surprised that they were on a trolley in the corridor for the night. I am routinely asked “is there no other way for a cancer patient to get into the hospital?”. This scheme does not change hospital admission policies, nor fund additional cancer patient hospital admission resources. When a patient is “triaged in to hospital”, in many hospitals that will be through the emergency department, although HSE hospital admission services vary around the country.

On Thursday, @thejournal_ie published an article (thejournal.ie/readme/oncolog…) I had written in support of recurring funding for the @hseNCCP, to enhance the NCCP’s capacity to deliver the National Cancer Strategy (#page=null" target="_blank" rel="nofollow noopener">gov.ie/pdf/?file=http…). During my outpatient clinic in Mayo University Hospital on Thursday, I had phone calls from @NTbreakfast and @drivetimerte for further comment. @rte kindly shared a statement from @roinnslainte, 15 minutes before the @drivetimerte call yesterday.
