@TraceyHerlihey I agree. Also struggled with statement that aim should be to eliminate "basic" errors (of course ideally they should be eliminated, but very challenging). Doesn't align with knowledge from human factors and is very failure focused rather than considering wider picture).

Is linear, complex and intelligent a better categorisation? Failing to de-ice a plane is not human error nor basic. It is a symptom of a complex system problem. Struggling to see the benefit of categorising failure as basic #Quality2024
