This week we piloted a PCN integrated virtual MDT. Bringing together care homes, GP’s, Geriatrian @EileenBurns13 , Nursing Teams @LCHNHSTrust, Pharmacists @LeedsGPPharmacy& Palliative Care to @stgemmashospice manage COVID 19 patients as a PCN team @LeedsGp @drgdubya @juliebeer6

My son Quinn answered a request from local hospitals for help making “ear guards” to take the pressure off medical personnels ears from wearing masks all day. He got busy on his 3D printer and has turned out hundreds to donate. We need help we can’t keep up to demand!

This is why we make a difference-my GPNs are contacting our high risk patients and today bought and delivered a bag of food to an elderly and isolated patient who had run out of food and had no supportive network to help. So very proud to be part of such a team @WeGPNs #angels
