How do you restore neural mobility for patients who have had back and leg pain? I tend to start with gentle ‘flossing’ style movements in supine then progress on to a gym ball glide. For runners I may progress into standing to replicate the swing phase and demands of running

I’ve often wondered if I’ve missed hip, pelvic, or femoral shaft BSIs in my patients, but a review of the literature suggests my theory that dancers don’t get more proximal BSIs, is probably correct (not including pars defects, just lower limb & pelvic)! A thread 🦴🩰 🧵 1/6

Gym ball squats are underrated! They’re a great option to strengthen the quads and load the patellar tendon. Especially useful if working into hip flexion is painful e.g. during a standard squat.

Consent ✅ High level 1500m runner Weekly volumes increasing Runing on beach / hilly terrain on holiday Increasing L quad (medial mainly) pain - 'bruised' feeling (thought was due to foam rolling) Then increasing sharp pain, even on walking Hop test +ive Resisted quads contraction irritable Femoral fulcrum test +ive with foam roller MRI - high grade proximal femur diaphysis stress fracture with cortical involvement (red arrow) & periosteal reaction medially at vastus medialis origin (white arrow) Adductor avulsion syndrome / 'thigh splints' can have similar clinical & radiological picture, but oedema & periostitis is too anteromedial for it to be adductor (which is moe linea aspera / posterior) x.com/akassarjian/st…

🦶High Ankle Sprains (ankle syndesmosis injury) can take up to 2-4 times longer to recover compared to lateral ankle sprains.🦶 The diagnostic accuracy of our clinical tests are of low sensitivity and specificity. 📊 It's recommended that a cluster of tests are performed. A positive result in the following 3 clinical tests should significantly increase your suspicion of a syndesmosis injury. 📈 Upon palpation, local tenderness of the syndesmosis ligaments was reported with a sensitivity of 92% and a specificity of 29%. 📊 The syndesmosis squeeze test found a sensitivity of 26%, with a specificity of 88%. A dorsiflexion-external rotation stress test sensitivity of 71% and a specificity of 63%. 🦵Additionally, Nussbaum et al. identified that an inability to perform 10 single-leg hops without significant pain should increase your suspicion. 🏥MRI is the gold standard of imaging with similar accuracy to arthroscopic findings. #sportsmedicine #physio #podiatry #anklesprain
